Our team can give you advice and support to help you manage a range of conditions. We can also support your family or carer to make sure you get the care you need after leaving hospital. We often help patients who have:
- been admitted to hospital after a fall
- been admitted to hospital with an upper or lower limb injury
- fibromyalgia
- mobility or social issues
- multiple sclerosis
- Parkinson’s disease
- rheumatoid arthritis
When you’ve been referred to our service, we’ll come and see you for an informal chat. This usually takes about 15 minutes.
During our chat we’ll discuss your history, your home situation how you came to be in hospital and make a plan for how best to help and support you.
We may give you exercises and equipment to:
- help restore your movement
- help you manage long term conditions
- improve your quality of life
- reduce pain
We may also refer you to other services or agencies who can give you extra support such as:
- Physiotherapy
- Podiatry
- Audiology
- Occupational Therapy
- Adult Social Care
If you’re staying in hospital overnight we might come and see you again to practice exercises and using the equipment we’ve given you. We’ll also decide whether or not you need extra treatment or rehabilitation before you return home. If you do, you may be moved to:
- a community hospital (see our Step Up Step Down service) or
- an interim care home health bed
