Our specialist nurses can help with things like:

  • catheter care and management
  • complex wound care
  • leg ulcer management
  • long term complex condition management e.g. heart failure, COPD
  • medication administration
  • palliative or end of life care
  • post-operative care
  • rehabilitation

If you’ve been referred to our service, we’ll arrange a time to come and visit you in your home or a local clinic to find out a bit more about your medical history and your current medication. Together we’ll identify your needs and create plan for your care.

During the appointment we may also do some tests such as:

  • blood pressure
  • blood tests
  • urine tests

We’ll also assess whether or not you need additional support or specialist assessments from other services such as:

Each time we visit you, we’ll make sure you have all the information you need, including our phone number and a copy of your medication charts.

When you’re health has improved, we’ll refer you back to the care of you GP.
